ANNUAL QUESTIONNAIRES
We have revamped our questionnaire process to make it more user friendly. We are happy to advise that we now have three options available for you to complete your forms. You can:
- Complete the form online and use the submit button to email it to us. Please ensure you print a copy of the form prior to sending it to us, as you will be unable to save this for future reference.
- Complete the form online, print a copy and send it in the post.
- Print the form, manually complete and post it.

What do you need to think about for year end?
This page highlights what you need to think about prior to completing your annual questionnaire(s).
Questionnaires
Annual Questionnaire - Business
If you are in a business complete this questionnaire. A questionnaire for each business or business entity that you own needs to be completed.
Annual Questionnaire - Personal
If you have personal income complete this questionnaire. One should be completed for each individual you wish us to prepare a tax return for. This also covers rebates and Family Assistance entitlements.
Annual Questionnaire - Primary Industry
To be completed if you are involved in a primary industry. A questionnaire should be completed for each business or business entity you own.
To be completed if you have a Trust that does not trade as a business. Please complete one questionnaire for each trust.
Annual Questionnaire - Rental Property
To be completed if you own a rental property. One questionnaire to be completed for each rental property you own.
